Thank you for signing up for our mailing list. Please find the depression questionnaire below.

Spend some time reflecting on these questions. Take your time, and notice if something intuitively stands out to you. Perhaps not all of the questions feel applicable to you. Let going through them start to help you map exactly what the issue you are calling ‘depression’ really looks, feels, and acts like.

1. What behavior accompanies your depressive moods? For some, this is binge eating or drinking, or sleeping in until noon, or withdrawing from social activities. Are there any accompanying repetitive behaviors that you find yourself in?

2. Was there an event (for instance a breakup, death, job change, etc) that worsened your depression or catalyzed the onset of your depression?

3. How connected do you feel to yourself on a soul level? Do you sense your purpose, your karmas, who you are spiritually?

4. Do you feel confident about your life’s direction?

5. Does your depression, moodiness, or anxiety come up randomly or can you notice some specific triggers? List the ones you notice.

6. Do you think down time and going through fluctuations is normal? How do you feel about not being 100% all of the time?

7. What emotions and thoughts are happening when the depression/anxiety is happening? Is there negative self-talk?

8. What sensations happen in your body? A heavy feeling? Fatigue? A blankness in the brain? Try to get in touch enough to feel what is going on in you.

9. Do you feel pulled on by other’s spoken or unspoken demands? Are you unable to set clear boundaries that others feel and respect?

10. How fulfilling are your relationships? With friends, family, coworkers, etc? Can you express yourself well? Do the people in your life really know you, deeply?

11. Do you feel resistant to trying new things? Is it hard to activate yourself towards your goals?

12. What are your fears?

13. When you aren’t feeling well, do you reach out for help, or do you keep to yourself?

14. What effect do your depressive states have on your relationships with loved ones?

15. When the cyclical down happens, is there anything you have found that changes it significantly? Or do you just kind of wait it out?

These questions will start to help you sense the areas that are the most intense in your depression. As depression at this level is usually a lack of self-care and self-awareness in some area of life, going through these can help already to shift your awareness towards being self-supportive. Working with a healer or counselor is always a good idea when you are not feeling your best.
